  • Re-create of a memorial or prayer card that provides the proper name of the deceased and date of death

  • Letter/note written by a staff member of a wellness care facility such as hospital, health intendance provider�south function, ambulatory habitation, etc.
  • Letter from an chaser or Executor/Administrator of the Decedent�s Manor.
